Nurses wishing to register with us can call or send us an email, with a forwarding address, requesting for a registration pack. This would be put in the post or sent via email as an attachment. Nurses are advised to come over to our office with the completed registration form for an interview. Requirements for registration: When returning the completed registration form, please ensure that you come along with the following documents: 1. International Passport showing you have a right to work. 2. National Insurance Number card. 3. N.M.C./U.K.C.C. Pin card. 4. CRB (Criminal Record Bureau)at enhance level, completed with us. 5. P45 6. Vaccination Record showing vaccination against Hepatitis B, Rubella, Varicella, T.B, small pox. We must see the full Blood Test results to confirm immunity 7. Two passport-size photograph. 8. Certificate showing that you have been trained as a Nurse/Healthcare Assistant. 9. Certificate of any relevant training/course attended. 10. We also require names, addresses, telephone no., Fax or email of two professional referees, one of them being your present/last employer. One of them could be a nurse whom you have worked with in a professional capacity and can therefore vouch for your nursing skills. 11. We also need a C.V. of your Full employment history, for the past ten years. 12. Your NMC Pin Card (For Trained Nurses Only) a) Statement of entry.
